The Origins of American Health Care

 The earliest traces of health insurance in the United States emerged with the inception of a prepaid hospital plan. In 1929, educators at Baylor University in Dallas, Texas, struck a deal with a nearby hospital, agreeing to pay a gradual stipend in exchange for comprehensive hospital care.

Initially established by Justin Ford Kimball, this initiative, as noted on the BCBC website, saw remarkable growth from covering 1,300 individuals to encompassing 3 million within a decade. The entity that arose from this endeavor would eventually be recognized as Blue Cross (later Blue Shield). However, the Baylor arrangement marked just the beginning. The enactment of the National Labor Relations Act compelled employers to engage in negotiations with labor regarding both wages and working conditions, serving as a pivotal moment for employer-based health benefits. During the wartime era of 1939-1945, government-imposed wage freezes led to heightened demand for group health care. With the inability to attract workers through increased pay, employers redirected their focus towards enhancing benefit packages. Government initiatives aimed at subsidizing healthcare costs gained momentum during the 1950s and 1960s, with the inclusion of disability benefits in social security coverage marking a significant milestone in 1954. The 1980s and 1990s witnessed a rapid escalation in healthcare expenses, prompting a shift in the majority of employer-sponsored group insurance plans from traditional "fee-for-service" models to more cost-effective "managed care plans." Managed care has become the prevailing model of healthcare delivery for most Americans. However, a significant portion of the population remains either inadequately covered, completely uninsured, or limited to high-end "Cadillac plans." While several Congressional Acts have been passed to safeguard insurance continuity between jobs, these measures primarily place the onus on businesses and fail to address the quality of coverage.
